Because the question refers to your "biography," you'll typically want to select five different scenes or moments from your life -- the "top five most meaningful scenes" -- rather than selecting one specific moment and dividing into five separate mini-stories. (The latter approach might work for some folks if there was some monumental single experience in their life, in which case five mini-moments from that one extended experience could be articulated, and might be used to parallel their broader life and biography, but most applicants will be better off with the "five totally different scenes" approach).
Keep in mind that this is more of a personal question than it is professional. One or two of the scenes you cover could be professional in nature, but what they're really looking for is you to open up and authentically share the five most meaningful moments from your life. None of them needs to be professional, so don't feel like you have to force that. Also, don't focus exclusively on achievements or positive moments. For most of us, the five most meaningful scenes from our lives include a mix of happy and sad, success and failure, triumph and challenge, and so forth. BE THOUGHTFUL AND AUTHENTIC.
As far as the song is concerned -- well that's part of the challenge and fun of the question. It will be up to you to figure out what single song connects those five separate experiences, however different they are. What song mirrors the ups and downs and authenticity of your journey? Why? Figuring that out is a very personal undertaking.
