Good medical care for all citizens is a national problem. Older people are running out of Medicare benefits and poor people are running out of Medicaid benefits. Also, it is not uncommon for people who do have insurance to run up bills above their insurance limits. But additional government funding is not the solution. Medical care is available at free clinics. So, the claim that people cannot get good medical care is wrong.

I cant understand how a is right!

Conclusion: Government funding is not the solution to the medical care problem. If we deny that government funding is not the solution to the medical care problem, medical care clearly can still be a problem. If we accept that government funding is not the solution to the medical care problem, medical care easily can still be considered a problem. Hence in both cases, medical care being a problem is compatible with the conclusion. The argument is that Government funding is not the solution to the medical care problem does not try to discredit that medical care is a problem, it simply states that additional government funding is not a solution to this problem. Hence D is incorrect.

One more thing-- The final assertion that "The claim that people cannot get good medical care is wrong" is also part of the conclusion, but nowhere does it say that medical care is or is not a problem. Medical care could easily still be a problem, or not.
