  Correct.   "based on not only X....but also Y" is parallel and is correct.

(B) not only on the number of questions answered correctly but also their difficulty level
 Wrong.  "not only  on  X... but also Y" is not parallel. The correct ones are: "not only on X...but also on Y" or "on not only X..but also Y"

(C) not only on the number of questions answered correctly but also their level of difficulty
 Wrong.  Same as B. "not only  on  X... but also Y" is not parallel.

(D) not only on the number of questions answered but also on their difficulty level
 Wrong.  Change meaning because  D omits "correctly" .

(E) on not only the number of questions answered correctly but also on their difficulty level
 Wrong.  " on not only X...but also on Y " is not parallel.

this questions uses the idiom

Not only X but also Y

X and Y need to be in Parallel form

only A and D use the correct idiom....but in D (there is no correctly- thus change of meaning)

TWO STRUCTURES ARE CORRECT HERE FOR IDIOM NOT ONLY ...BUT ALSO.
Those are in option A and in option D.
  on  NOT ONLY.....BUT ALSO ==>OPTION  A 
NOT ONLY  ON  ......BUT ALSO  ON  ===>OPTION  D 
 
but option  D  CHANGES THE MEANING  by omitting  the word  CORRECTLY .

its not only about parallelism but also about meaning (BTW i just used "not only but also !! ) .meaning has to come out unambiguously . in D the fact that questions are to be answered correctly is missing and that in fact is very important for keeping the meaning intact

SO it reads  Based on on their difficulty level

For D
Based..
 on the number of questions answered 
 on their difficulty level 
Both usages are correct & parallel

A is also parallel, in D correctly is missing and that causes loss of meaning. What do you say??

could not decide between A and D, but then noticed that D does not have "correct", so A must be the right answer.

Actually I think A is not parallel, feel free to correct me. I think D is parallel since not only "on".....but also "on" and I heard that grammar always comes first and meaning second.
Again if I am wrong please correct me.
