Remove A, B, and C for wrong comparison. D twists the meaning that the elders respect in most western countries. E is the correct choice.

In Japan elderly people are treated with far greater respect than most Western countries.
For me it helps to flip the order of the words: Elderly people are treated with far greater respect IN JAPAN than IN most Western countries.. So, to maintain parallelism with "in Japan" we need an option, which uses "IN most Western countries" phrase.
Only D and E do so: E wins as proper usage is "they are (treated) in most Western countries", not "they do (treated) in most Western countries". Ellipse in E is correct.
