There are no big-name companies in the country he's coming from, it's pretty much impossible to have a real career progression there. The best he can do is keep working in the small web startup, but I don't think another year is going to add much value to his app/credentials. They've already built things that gained enough local attention (e.g. evaluated by the prime minister), so this can be incorporated into the 'impact' aspect.

Yes, in my experience, admissions officers are more flexible with applicants from underrepresented countries. However, they will still want the applicant to bring value to the classes, so your friend will need to demonstrate great insight into his country's workings in his application to prove that he will do so.
