As far as I know, the schools only see your last three GMAT attempts so I wouldn't worry about it too much - but with your score I'd hope this is the last time (your score is fine).
As for your dyslexia - I assume the test admins would've made some accommodations (i.e. allowing more time for you, etc.)? In any case, it's really up to you whether you want to disclose it or not in the additional info section - because it's not going to be a negative or positive to disclose it, especially if your main point is "I have dyslexia, but don't worry, it won't affect my ability to keep up in classes nor have I ever used it as an excuse before in my life". Saying that is the same as not disclosing it at all. Again, it really depends on why you'd be disclosing it in the first place - if it is to explain a lower GPA or something that, then sure, but otherwise it's really a personal decision.
