I have given GMAT Focus Edition 2 times, and have never appeared for GMAT/GRE/Executive Assessment. So, here my answer should be 0 or 2?

GMAT Focus is simply a new version of the GMAT, thus it would be irrational to consider it as anything other than GMAT.

Yes, it has to be 2 in your case.

Your answer should be 2 as you have taken the GMAT (Focus Edition) twice. GMAT FE will soon be called GMAT anyway.
