To answer your questions:
1) Given your age and level of full-time work experience, I actually think you should consider applying sooner rather than later. Just this week I spoke with an applicant who said, "I kept waiting to apply to business school, because everyone kept telling me to get another year or two of experience before applying, and now I learn that schools are trending younger and that I'm too old." This is mainly just a handful of the top schools right now, but you don't know when the other top-ten schools will decide to go the same way. You have enough experience now that I think you should consider applying this fall. Anyway, assuming that you can passionately and believably communicate your desire to help others (i.e., not making it sound like something you're just saying to get into b-school), then I think you have enough of a shot that it's worth applying to a top-five school, but you should also consider a few lower-ranked schools, just to improve your overall chances.
2) I don't think that raising your GMAT score will help you much, UNLESS you can significantly improve your verbal score. If you don't think you can, then you should probably just focus on the rest of your application.
