MD Internal Medicine, need help

Hi there,
I am Male/29/Indian.
I am currently pursuing MD Internal Medicine from a very prestigious college in south India and will be finishing next year following which I am planning on applying for MBA.
Following is a brief description of me.
I finished my undergraduate training from a very prestigious med school in north India followed by 1 year of internship. I took USMLE and cleared the exams with top scores. Following this I was working as a research assistant at one of the universities in the US. I joined residency in the US in Internal Medicine in 2010 but I was forced to resign in 2011 due to my own follies (which I realize now in hindsight; it took me quite some time to get to terms with the fact that it was my own fault). This was a major setback for me both personally and professionally. I was a very humbling experience for me.
I gradually realized that I had an inclination towards management than medicine. I discovered my true passion during stay in the US which was food. So it was clear to me that I had to get into the food industry. However, having started something, I wanted to finish it so I came back to India and joined my present MD Internal Medicine program. My current training has been a wonderful experience and has given me the opportunity to develop great interpersonal skills and deep interpersonal relations with both my colleagues and patients.
What excites me most is food entrepreneurship, however, having said, I feel that I would enjoy any managerial job more than medicine.
I am an amateur photographer and have a keen interest in food both trying different cuisines and experimenting in the kitchen. I love traveling and have traveled to more than 15 different countries. I love the great outdoors and have trekked to several remote regions of the Himalayas. I know 4 languages including English, Hindi, Punjabi, Kannada.
I have been actively involved in philanthropic activities right from my undergraduate days including free health camps for the poor people organized by my hospital, working in a charitable/free hospital, administering oral polio immunization as a part of WHO Pulse Polio Program in India during my undergraduate training.
As far as my leadership goes, I was a badminton captain during my undergrad years and I am currently heading a research project involving >300 patients in collaboration with Indian Institute of Science. I have been actively involved in teaching undergraduate students during my MD. I have been actively involved with the activities of the Lipid Association of India.
My question to you is that if I get a score or >700, do I stand a chance of getting into a top b school and how will my professional setback play out in my application?
