So far, I have practice 6000+ questions. Many of quant questions come from gmatclub.test and Math revolution. It is strange because I only skip 2 math questions.

This does not paint a complete picture. Did you take all the tests in the same scenario?
Generally, people tend to pause the exam or take longer breaks during mocks. Hence, it could not be a real indicator.
Also, stress and time-management during the exam are very important factors.
If you could brief on what happened on the D-day, then we can possibly try to isolate the case.
Don't be disheartened. Take a short break and start fresh.
On the exam day, I have no problem with time on quant because there are fewer hard questions than prep tests from MCAT, gmatclub,...
The order is Quant Verbal IR AWA
On the exam day, I do not feel stressful or tired , and I am concentrated but I completely do not understand, or know what to do with the verbal questions. During the prep tests, I only pause for quant, and skip many Integrated reasoning questions. I practice AWA everyday.
I suspect that I keep making wrong answers on easy questions. In fact, I give wrong answers on 300-400 questions as often as on lvl 700 questions.
