Define something not as x but as y, is idiomatically correct and the structure maintains parallelism. One more parallelism checked here, i.e. impairment that presents... and impacts....

B. not to be a form of childhood schizophrenia but rather a spectrum condition with degrees of impairment presenting - breaks both parallelism of the sentence

We have successfully eliminated four answer choices. Option A is parallel and uses the correct idiomatic structure. Option A is the best answer.
