Thanks for sharing! Sounds like you're goal is to switch careers, which is more geared towards a full-time MBA program. One thing you'll want to consider is how everything will play out for you upon graduating from a FEMBA. It's definitely still possible to find a new career opportunity through on-campus recruiting, but it's just not as likely. So, be sure to do the due diligence and ensure that there is a path for you to switch careers through that program.

One thing I'm not seeing clearly (these forums aren't the best place to go in depth about this anyway) is your story. How are you connecting the dots from your previous work experience to your goal of being a supply chain consultant? What skills are you leveraging from one experience to the next? How does your experience at a startup play in to this? These are all things you'll want to incorporate in your application.

Your involvement is super light, so that will be a bit of a flag. One way to compensate is to demonstrate your leadership and volunteer work at your employer. These are all things that show your desire to go above and beyond the call of duty to make a positive impact.
