AcademicsYour GPA is ok but a bit on the weaker side. Probably a side effect of your accelerated degree. It's a plus to finish a 4 year degree in 3 years BUT there was clearly a cost. In addition to your credits, you were probably pulling in 18 hour semesters most of the time? If not more? That's a lot. But you went to a top bschool feeder school (how about 'em dogs?!) so that'll help.
Your GMAT is good too but not amazing. Below average for some of the schools you listed, and above it for others. Solid split though and great quant!

ExperienceWhile I was reading your post, I liked your diversity of experience until you mentioned the end of your first consultancy job. So on one hand, you've done some interesting work in both a tactical / operational role but also in client facing work. But let's talk about the 800lb gorilla. You were fired for performance. I can't say how much was your doing versus that of others because you mentioned the whole team but that's really not a discussion for a forum. You probably have a few interesting experiences to pull from but answering the why you left that firm question is going to be a serious flag.

InvolvementIt's good that you're involved, but you're really not that involved. And your involvement is more of an activity than something with scope. Also, you've been doing both for just over a year so they're newer activities as well. Like I said, I'm glad you're involved but it doesn't jump off the page at me.

GoalsYour goals make sense and are realistic & ambitious - just make sure you articulate the "why" very clearly!

Overall & SchoolsThe schools you've targeted range from fairly aligned (e.g. Duke) to stretch (e.g. Kellogg) but you don't have any safeties. You have most of the basic things you need but, like I mentioned, there are definitely red flags in your profile. I'd say expand your list downward a bit too.
