Thanks for your post. MiM in some ways is less onerous than MBA application process from an admissions standpoint, and in general schools will be more interested in your academic preparations. Schools will definitely take the rigor of your program into consideration as well as the reputation and ranking of your school, so when they convert your GPA to the 4.0 scale, rest asssured it will likely be higher than you think as you indicated. I see no reason to build an alternative transcript.
While MiM programs can be a bit more lenient on things like GMAT scores since they are not as heavily scrutinized, for example by the rankings boards as the flagship MBA programs, you are wise to still shoot for a 7-handle on your score. Can only help and would balance out any long term potential negative on the off chance your GPA does not impress them.
As Founder and CEO of a startup organisation which is gaining momentum and making profits, you will be viewed as innovative and entrepreneurial. You can cast it as a hobby or side endeavor to keep it from being considered as a work experience if that is a concern. You may also need to inquire how they will view the internships--just make sure you clearly indicate on your resume as such, also listing your company in an additional info section vs. professional work section.
As for all the extracurriculars, you can list in comma-separated format under the college section as things you participated in during college. no need to make it an exhaustive list--can even say "heavily involved including participation in..." then list what is most important to you or what you feel is most impressive to the committees and most relevant to your post MBA goals.
Your school list seems fine with a good mix across the rankings. I always reccommend going to school in the geographic region where you want to end up working, so your network is large and close by. In most cases this means going to a school within a 6-8 hour drive of the area where you want to work.
