Thanks for sharing your information with us. Glad to hear you're planning to retake the GMAT. Anything you can do to boost your score will help your chances. That said, I think your target schools need to be adjusted a bit. Darden, Fuqua, and Ross are going to be stretches for you. As part of a diverse application strategy, I'd recommend targeting 1-2 max out of this list for this cycle. With a boosted GMAT, I think UNC and Texas make sense, but that will be highly dependent upon the quality of your application (i.e. how well can you craft your story?)
It seems like you have some solid leadership experience, however I feel like you're selling yourself short. Of course, I know we're in a forum and it's tough to get into the details of your leadership and accomplishments, but I mention this only to guide you to ensure that you provide details of your experiences so that you can stand out amongst your peers.
You have a good starting point on your goals, but they could be more specific. What kind of consultant do you want to be? Industry categorization is one thing, but if you're looking to be a management consultant, top firms will start you off as a generalist before allowing you to specialize. Or, are you looking to focus in a particular function of consulting - IT, finance, ops, etc? What do you mean by "leadership roles" in industry? You'll need to get tighter about expressing your goals post MBA and how you think a given program can help you.
