thanks this is really helpful. What about the component of the recommendation that asks the recommender to comment on the applicants "ethical" behavior.

Are they looking for examples here, of when integrity / ethics were tested and how the applicant responded?

What if there were no real "on-the-job" experiences that tested the ethical behavior of the employee/applicant? Does the recommender comment based on their own personal assessment of the applicant's ethical values? Saying "I think she is very ethical and trustworthy. She treats everyone fairly." doesn't seem to do much to SHOW the ADCOMs you are ethical.

In all cases, the recommender will comment to the best of his or her ability and provide the best example or substantiation possible. If you weren't tested on the recommender's watch, then the recommender will go by personal assessment.
