Based on the information above, how would you assess my chances, to these college? Is my record of Commmunity Service sufficient?

You are aging out of the HBS and Stanford, certainly, but the other programs are a bit more flexible about age (INSEAD in particular skews higher). You may be interested in the MIT Sloan Fellows program or Stanford's Sloan Master's program, but these don't offer the same intense recruiting options that the traditional programs provide. My biggest concern is that you may not have the leadership and innovation in your company that they are looking for (it may be there but you didn't mention any large initiatives in your post).
Your extracurricular involvement in sports and events with your company sounds great. The undergrad leadership is going to be less relevant because it was so long ago.
