B) and extended national testing programmes that monitors wild birds and domestic poultry for the flu virus --- programmes that monitors --- SV error.


C) and extending to a national testing programme to monitor wild birds and domestic poultry for the flu virus --- extending to is unidiomatic

D) and extended programmes to monitor wild birds and domestic poultry on a national level for the flu virus--- programs to monitor on a national level is not as concise as national testing programs.

E) and extended national testing programmes to monitor wild birds and domestic poultry for the flu virus --Correct choice.

Sentence needs the construction : X, Y, AND Z
For parallel construction, 'Extended' is the correct form of verb. ( A and C are out)
Programmes is plural, requires plural verb form. ( B is out)
D has an awkward construction.
Hence, E is the winner.

Explanation:

“New measures include... increased surveillance, requirements, and extended testing programs so A and C are wrong. ”... “programs that monitors” subject-verb do not agree, so B is out. D is wordier than E, and 'national testing programmes is better than 'on a national level' because the modifier national level should be close to the noun 'testing programmes'.
