As per my understanding:
A. she was by far the most unique student at the entire academy. - wrong
Error Analysis :
" Unique" it self is unique word , which does not need most or more to it . It will be redundant.
best in the academy itself conveys the meaning that best in the entire academy , here entire will be redundant.
B. she was by far the most unique student of the ones at the academy. - redundant
(Most and Unique) and also says one among most unique ones which is wrong . Unique is only 1 .
C. she was the most unique student by far at the entire academy. -redundant ( Most - unique)
D. she was by far the most unique student among the entire academy.-redundant , also among the entireacademy -- students
E. she was unique among all of the students at the academy.-Correct

"Most unique" is always wrong. "Unique" is one of those special words in the English language that should never be modified. Why? Because "unique" already means the only one/having no equal/unparalleled/sole. How can something be MORE than number one?
