The sentence becomes fragmented, so eliminate A & B.

Note, the tone of the author of the sentence, denotes an action completed prior( failed hiring practices) to some past point of time implied(during review) The non-underlined portion of the sentence, "none should be considered", makes it clear we should use subject verb agreement, "emphasized"(past event) to fix the modifier/fragment sentence issue. This review event which is already past, refers to the prior past event "failed to account", warranting it to use "had failed" (past perfect tense) .
