Maximum commercial efficiency requires that large swathes of crops are grown in monoculture, under which conditions they become vulnerable to disease and premature death.

This question has 2 types of issue : Subjunctive + Pronoun agreement.
Pronoun Agreement: they vs it, they refers to Swathes, which is plural so they is correct. Eliminate B,D,E
Subjunctive: now the trigger word for subjunctive here is requires, as per multiple sources(MGMAT, Egmat) we can use both infinitive or bare infinitive form of the subjunctive verb with the subjunctive indicator require
so both to be grown or are grown is fine, then why is C preferable over A ?

Subjunctive requires the use of 'be' in passive construction where some form of 'to be' will be used otherwise.

Passive Construction : X are grown by Y.

In Subjunctive: He requires that X be grown by Y. (Correct)
not
"He requires that X are grown by Y." (Incorrect)

- The correct form for the bossy verb "requires" is either C or D.
-"They" vs "it" split: we talk about "crops" here, hence "they".

This question has two major decision points: 1) word choice around the subjunctive mood and 2) pronoun agreement. Taking a look at the end of the underlined portion, you have a choice between “they” and “it” for our pronoun. Because the pronoun refers to the plural “crops,” the correct pronoun must be “they,” meaning that you can get rid of answer choices (B), (D), and (E).

The difference between (A) and (C) is that one requires that crops “be” grown, while the other requires that crops “are” grown. The GMAT is testing whether test takers are comfortable with using the subjunctive when discussing opinions, beliefs, purposes, or desires. The construction “require that” must always be followed by the infinitive form of the verb (in this case, “to be grown”) with the “to” removed. The correct answer is therefore (C).
