How is hardest determined?
Is it just raw % of test takers who get the answer wrong?

If so is there a certain number of attempts required on the question for it to make it in the list? I imagine a question with one wrong answer and no right answers [0% correct responses] wouldn't immediately appear at the top of the list.
Additionally does it factor in time taken on average before responding into difficulty?

Thanks for any insights on this.

It's a weighted average which depends on number of parameters such as the number of timers (attempts) for a questions; attempt time; ... Also, there are the minimum number of attempts requirement and the minimum attempt time requirement for a question to get the automatic difficulty.
