C is clear in meaning with respect to context of sentence and restrictive modifier to "many athletes".

In A, "at one time" is creating problem there.
In B, "that due to ..." seems wrong.
In D, "who due to catastrophic injury might have at one time lost their ability" is seems wrong too.
In E, "who, due to" is clearly wrong.

C is correct.

+1 for C.

A) that might at one time have lost their ability to participate in their sports due to catastrophic injury
B) that due to catastrophic injury might once have lost their ability to participate in their sports
C) who might once have lost their ability to participate in their sports due to catastrophic injury --> Correct, rectifies diction error and pronoun ambiguity
D) who due to catastrophic injury might have at one time lost their ability to participate in their sports
E) who, due to catastrophic injury, might at one time have lost their ability to participate in their sports

Hence, C.

As a result of advances in biomedical engineering, many athletes that might at one time have lost their ability to participate in their sports due to catastrophic injury now can pursue their sports again, even if not as professionals.

A) that might at one time have lost their ability to participate in their sports due to catastrophic injury - That cannot be used to refer to people

B) that due to catastrophic injury might once have lost their ability to participate in their sports - That cannot be used to refer to people

C) who might once have lost their ability to participate in their sports due to catastrophic injury - Correct- modifier "due to" is placed next to the noun phrase "ability to participate in their sports" it is modifying

D) who due to catastrophic injury might have at one time lost their ability to participate in their sports - Modifier "due to" is not touching the noun phrase it is modifying.

E) who, due to catastrophic injury, might at one time have lost their ability to participate in their sports - Modifier "due to" is not touching the noun phrase it is modifying.

In option D & E, is it really important for hte noun phrase to touch the modifier. I mean should this always be the case?
