Here the subject 'financial information' is singular right? If so, it should use 'follows' than 'follow', isn't it?

I think its C

Bankers require that the financial..... be complete and (that) a prescribed format is ....

i think subject here is "bankers" .

"require that" implies subjunctive mood. So "follow" is correct.
A should be the correct answer.

I think C should be the one.

"bankers" is the subject while "financial information" is the object.

REQUIRE is a special verb that can take either a Command Subjunctiive(THAT + Subject + Verb form) or an infinitive.

In the statement, Bankers REQUIRE THAT , it is following a pattern of command subjunctive verb mood.

A - My choice.
B - is complete. For command subjunctive ONLY "BE" is allowed, No other Be form such as is, was, were is allowed. Out of the game.
C. be complete and a prescribed format is followed. Here AND is a parallel marker, and the phrases before and after AND are expected to be parallel. A presribed format is not parallel to BE COMPLETE. Out of the game.
D. to be complete - to is not the correct usage. There should not be TO for command subjunctive. Out of the game.
E. be completed. Always the verb should be bare form of the verb(complete) but not completed. Hence out of the game.

This is a case of subjenctive verb hence A should be correct.

A is the right answer. The idiom goes like this "REQUIRE X Y"....... require X to be Y is not the correct usage.

It is A. It seems this is very easy question if you know the rules. Please refer Manhattan GMAT.

require.... (A (ask), B (bag), O (order), P (prefer), U (urge) R (require) (ABPOUR): If you come across any of these verbs
then they need either "bare form" or "infinitive form" after that.

Here it requires "bare from" because, construction of sentence is as follow,

require that "subject" "bare form of verb" and "bare form"....(if that is not there then we need infinitive form).

Some thoughts:
1. Require that X be Y:
Example:
The law requires that they be regular.
2. Require X to do Y:
Example:
The law requires the women to do their duties.

Now, come to the sentence:
the structure of the sentence is "require that X be Y". So, B and D are out at a first glance. C is out for using "prescribed", and E is out for using another clause.
Answer is A.

"require that X be Y" is key.
"Bankers require that the financial information ......be complete and ...follow..."

The best response in line with the correct idiom is option A.

I believe you need to use be and follow, because the active verb is require. It would be incorrect to use is or follows.

Information cannot folllow a prescribed fromat. The doer is not presents in the sentences. When the doer is not present, passive voice is perfect. Cannot be A as an OA.
