Not mandatory. If the medium of instruction was english - it will be not required

Can someone confirm if they received an application fee waiver recently?

The same is stated on the test column

If English is not your native language and/or you have not earned a degree from a university where the official language of instruction is English, you are required to submit official scores from the Test of English as a Foreign Language (TOEFL) or the International English Language Testing System (IELTS) exam

Yes, Ideally you should. Shows growth evidently.

When you fill out the name, it sends out a mail to the recommender asking them to fill in. Either confirm from your recommender or contact AdCom.

Cornell is yet to respond to the initial email confirming my application fee waiver. I'm almost panicking now as deadline is Tuesday.

I was offered an application fee waiver for attending an event, I only need them to effect it so I can submit.
