For MIT interviews, since the adcom has already read everything, I have to come up with examples that are not mentioned anywhere in my essays?? Even if they fit perfectly??

Yep! That is correct. They are pretty strict and straightforward about that. They explicitly state that at the beginning of the interview, as well. I had 4 good examples through my essays and cover letter but I also came up with other examples that could be applied to a variety of behavioral questions when preparing for my interview. I probably used 3 detailed examples to answer my interview questions, because each example has a plethora of follow-up questions.

But they literally say "I have read your application, so I want to hear new examples"

I just noticed that I didn't 'waive' my right to view the recommendations. Does that negatively impact the application ? btw, count me in for R1.

That's a federal legal req, as you, as a student/applicant, have access to ALL student records. Recommendations are part of these records.

It *shouldn't* affect how an adcom looks at you, but thee adcom can see your answer.

It definitely affects how they look at the letter. They expect letters that can be viewed by the candidate will not be as candid. Legal rights have no role in this and they are within their rights to look at it differently if they believe it will not be as honest. Any adcom officer will tell you that. Maybe the applicant could call them and tell them to make a note that you meant to waive it (if it's not too late)
