Honestly this transcript form is strange - it says "complete above" and there's nothing above; I attended at least 4 different universities in three countries - how am I supposed to send them return envelope - this will never arrive back to me snail mail.... i bet we can use electronic transcripts these days.... will ask them. I had my transcripts translated and asked an AdCom if I could just upload those; he said that of course as it would not make sense to rewrite all the info myself if they were already translated. Just in case is useful for anyone.

Does not really matter where you are educated - Registrar's all over the world would know what to do with this. But (1) snail mail takes for ever, (2) may be not all schools rank their students (I am not aware to have had a rank ever. But these are just procedures....

In case our transcripts are already in english we don't need to submit any additional forms (apart from the scanned transcript), right? Am I missing something? Or is this form something everyone is supposed to fill and submit?
