As best I can tell, the form is new as of this year. Other than simply returning the form indicating that one wants to be considered for a scholarship, all of the 'fill in' sections are optional. Questions include:
-- The first question asks, with a 150-word limit, about one's "specific circumstances" that might have a bearing on one's "scholarship request". They offer a qualifying statement that although their scholarships are not necessarily need based, they're "sensitive" to personal financial circumstances. Other than financial need, what would one write here that isn't already in the application. Applicants have already spelled out as much as they could what they'd bring to Yale SOM. What would one write here that would be helpful for getting a merit scholarship?

optional question is how one plans to finance business school. They're asking for a detailed plan, with all the components (savings, loans, family support) adding up to 100%. They ask for one's spouse's income? How would this affect merit-based scholarships? If one has been diligent to save enough for business school, does that hurt one's chance of getting a scholarship?

optional question asks to what other schools one's been admitted to, up to a maximum of three, and what scholarships have been offered. I imagine having some good scholarship offers has an impact on what Yale does, but that could be from all sorts of directions. It could help, if they think they need to offer you a financial incentive to attend. Or maybe if they think they can't compete with another school's offer, they could offer you nothing. And maybe they use another school's offer as a cap/maximum to their offer, as they may have gone higher if they were in the dark, in terms of with whom they're competing.
Needless to say, this reflects pretty poorly on them. If they want to base their scholarships on need, then just say so and ask for the relevant info in the application. And what if every school starts taking this approach, especially waiting to hear what other schools offer? Does it get to a point than no one offers merit scholarships until very late in the decision period? That's the last piece of this. They won't extend offers until early April, when deposits for some schools are less than two weeks away. How does one arrange admitted student day visits that late in the cycle, waiting to see if they're necessary depending on whether Yale is in the mix or not? They've had months to make their decision, but want to give the applicants two weeks. And that's my TED talk.
