That's a good place to be in - careless mistakes take the longest to root out or at least they did for me and require a different approach from how to learn material.
You will need to slow down significantly in your practice and pick out every single careless mistake you make and then devise a plan how not to make them. This is a highly personal plan but to give you some ideas - e.g. pause before you solve every question in quant, recognize when you are most likely to make mistakes and double-check your calculations, re-read the question if that helps, etc.

In my experience, the careless mistakes require a mental checklist that you keep in addition to any other material. This will slow you down but it will make it more accurate and it is critical to know yourself and to know when you are likely to slip up so that you can anticipate it and put measures in place to avoid it.

I also had a doubt in the study plan - if i dont hit 80% accuracy on easy questions, what should i do? Review and solve new set of questions again? And like keep solving more & more questions till i hit 80% accuracy?

I feel reviewing the mistakes of your question set is more important and then you can practise 5 sets of 10 questions, in untimed manner and making sure to get it right.

Spend atleast 5 mini reviewing each question. Solve it independently again. Try multiple approaches especially for quant.
