Obstetrics is singular, so eliminate B, C, and E.

Obstetrics rates --> Obstetrics cannot rate, main verb is missing. So D

Obstetrics, with its unusually high malpractice insurance premiums, rates among the most expensive medical specialties for doctors to practice, eclipsed only by various forms of surgery.

A. rates among the most expensive medical specialties for doctors to practice, eclipsed only -> Correct; eclipsed only by ... is acting as a Noun + Noun modifier and can modify noun or clause.
B. rate among the most expensive medical specialties for doctors to practice, only eclipsed -> Obstetrics is singular as is suggested by its in non-underlined part, hence rate will be Incorrect
C. has a rating among the most expensive medical specialties, only eclipsed -> only eclipsed change the meaning, has a rating is awkward - Incorrect
D. are among the most expensive medical specialties for doctors to practice, eclipsed only -> are is incorrect as is suggested in option B - Incorrect
E. have been rated among the most expensive medical specialties for doctors to practice, only eclipse -> have been plural is Incorrect

I said D.
A: 'rates' is singular. This is against 'obsterics' which is plural.
B: wrong use of 'only eclipsed'. 'only' should follow 'eclipsed' since this sentence refers to various forms of surgery topping Obsterics
C: 'has' is singular. This should be plural
E: Wrong use of 'have been'. This should be simple present.

OFFICIAL EXPLANATION

The sentence as written is pretty clear, but we should always go through our answer choices to make sure we haven't missed something.
Choice (B) changes the verb "rate" to have a plural subject; is "obstetrics" a singular or plural noun? Since it refers to a single field of study, it's singular, but if we didn't know that, we could also see from the pronoun "its" that it is singular; if it were plural, the pronoun would be "their."
Choice (C) is unnecessarily wordy, and places the word "only" in the wrong place.
Choices (D) and (E) use plural verbs, so neither can be our answer.

The correct answer is Choice (A).

Good question, Hema15. The reason obstetrics is plural here is because the sentence describes the discipline as rating among the most expensive medical specialties. That is, obstetrics can only be one medical specialty or category, just as surgery is understood to represent one broad specialty. Otherwise, the sentence would have to outline some other subspecialty of obstetrics, such as gynecologic oncology or reproductive endocrinology, and discuss the ratings a little differently. This issue is kind of like tackling a question about moose or deer. The context reveals the necessary subject-verb agreement.
