Hi,
I wish to send in my application to colleges in the United States. I had a few questions regarding the same:
I am an Indian and my graduation and post graduation degrees were in English as the primary language of instruction.
To apply at INSEAD, Columbia, Stanford, Kellogg, London Business School - do I need to give my TOEFL? Also, my mark-sheets show marks in numbers and percentages. Do I need to get this converted into GPA? What is the process for the same? How do these transcripts get generated?
I have 7 years of work experience across 3 organizations - do I need employment letters from each of them?
What about Letters of Recommendation - I do not wish to ask my current line manager, can I ask any of my previous ones in the same organisation?

Hello there!
So first, as an Indian unless you studies in US/UK/Australia you DO need a TOEFL!
You do not however need to get your grades converted. You send them as-is, and the B-schools know what to do with them
For your LORs, you can choose whoever would be best for you, you do not have to choose your current supervisor, although it is advised that you have one recommender from your current workplace.

A slightly different take on things here Sam. Many schools do exempt TOEFL if you can show the medium of instruction was English - in many cases, an English transcript suffices. The actual deal here though varies from school to school and hence it is best to get this double checked with the schools you are applying to.
You will not require the employment letters now, but many schools will ask for it after you've been admitted as part of the background check.
It is preferred to have 1 reco from your current manager. Some schools insist on it and may need you to provide an explanation, some are ok without it.
In short, like life, there are all sorts of flavors here - no cookie cutter answer
