This same thing happened to me. Received an email (and portal notification) Tuesday night stating that the LOR has not yet been received and is needed by the 15th, even though my supervisor was pretty sure he had submitted it on Nov 28th. And the portal said "link forwarded to supervisor". I did not talk to or email PGP about it, as thankfully was able to get in touch with my supervisor immediately and requested him to fill it again on the original link he had received. The portal immediately updated to "Recommendation received" and I got the confirmation email the next day. I thought this might have been a miss from my supervisor’s end, but it looks like some sort of a widespread thing if others are facing this as well.
