Since the first organ transplant was performed in the United States in the 1954, a serious problem faced by transplant recipients have been the possibilities that its own body will try to reject or destroy the transplant.

A) have been the possibilities that its own body will
B) are the possibilities that their own bodies will
C) have been the possibility that its own body will attempt to
D) have been the possibility that their own bodies will
E) is the possibility that their own bodies will

This is just all sorts of wrong. This correct answer requires a present-perfect tense "has been" as the problem has persisted since 1954 and still persists even today. Goes without saying that there is a SV agreement issue with the OA. "A serious problem" is a singular subject that requires a singular verb, not a plural verb like "have been".

Either the source/OA is wrong or there has been a mistake transcribing the question.
