I would advise against writing your reco. Your language/ style is bound to come out.

I'd suggest you go through the recommendation letter, and put down, in bullet points, characteristics/traits with 1/2 supporting examples for each question. Don't write out full sentences, just short points. Then, forward this to your recommender and ask him/her to complete the form.
If your recommender still insists that you write the whole thing, forward the bullet points to a colleague and ask them to complete it. Then, forward the completed reco to your official recommender.

Yes, but in the real world, you might have to write your recommendation. So as wisely stated above, you don't want it to sound like you. So make some stylistic choices and changes if you have to.

One strategy that I was advised is to write out phrases and descriptive bullets about the topics and experiences you want the recommender to cover in a different language and ask him to translate it literally in english in the reco. That way your words wont come off as yours.

But be aware that: your recommender must be willing to put in that effort & time for you, which I doubt is the case right now for you, unfortunately.
