My answer is (D). It took me 44 seconds.
(A) Note the use of "only three major accidents", which indicates that the World Nuclear Association is pleased with such a record. Therefore, "but" is inappropriate since the next clause paints even rosier picture for the future.
(B) "and" is appropriate, but we prefer "and that" to be parallel with "that" right after "stated".
(C) See (A)
(D) Fixed the issue in (B) without introducing new issues.
(E) "so that" indicates "cause - result" relationship, but no such relationship exists.
