Option E is the best. There is no ambiguity. Other choices are awkward. There is no time frame or location being mentioned . Please give me kudos. I need them badly.

Correct choice is E. ‘While’ acts as a subordinating conjunction and in this sentence by using ‘while’ the subordinate clause (sparing others) is linked to the main clause

Police are mystified by the serial murderer and have no explanation as to why he attacks some of the prostitutes with whom he has contact when he spares most others.

(A) when he spares
(B) where he spares
(C) where sparing
(D) when sparing
(E) while sparing

MANHATTAN REVIEW OFFICIAL EXPLANATION:

‘When’ refers to times and ‘where’ refers to locations. Both are inappropriate here. ‘While’ introduces a contrast. There is a contrast present in this sentence. Choice E is the correct answer.
