Hi there!
Thanks so much for sharing the details of your profile. Overall, I like your profile a lot. You have a lot of things going for you. Your academic profile puts you in the conversation at a lot of programs. While your Quant score on your GMAT is a tad low, for your application pool, you're in a good spot. Assuming you're applying in R2, I wouldn't worry about taking it again. Further, your engineering degree will give the AdComm the confidence that you have a strong analytical foundation. In terms of your UG GPA, do you have a class rank that explains where you fall against your peers? If not, I still wouldn't be too concerned. If your school is as competitive/rigorous as you say, the AdComms will take that into consideration.
Your work experience is strong. A lot of activities that will appeal to any AdComm and the promotions definitely help. Do you formally manage a team with direct reports? I'm assuming you had a fair amount of responsibility and accountability with both personnel and finances as a Senior Analyst. While the decision to leave your firm to start your own consultancy isn't necessarily a bad thing, you'll have to explain this decision fully. You'll want your recommendations to be strong, so with some guidance, we could definitely find the right people to write your letters. One area that is a bit light in my mind is your involvement. The activity that looks intriguing is the work with the Elderly Care Centers. How big of an initiative was this? How much time did you dedicate to this? Over what period of time? As far as your goals go, spend some time honing in on what the role and industry you want in the near term. You'll need to build a compelling case that aligns with your story and to do that you need a specific goal to tie it to.
For your school selection, I like the schools you're targeting, but you'll need to prioritize based on your short-term goal. If supply-chain is where you truly want to be, top schools in the US include Sloan and Stanford in the top 10 as well as Tepper (CMU). Sloan and Stanford are stretches, but you should give at least one of them a shot along with the others you're targeting globally.
