It is vs it refers

Usually for terms reference is used

Straightforward
(d) imo

Also in
A- proceeding to one's plans is incorrect

IMO D.

D clearly states the meaning of the sentence.

Rest fail to do so

answer is Straight D as a term can refer or explains but not with is or was verbs can be used.

I am confused in between D and E. Why is D preferred over E?

In my opinion, this is a very poorly written question, and not very GMAT-like. I wouldn't recommend spending too much time on this one.

Hi all, E is wrong because to proceeding is unidiomatic. It should be written as to proceed.
