A) If the medical bills are paid by government for all regions int he country of Bellaria (wistar is a part of it) then everyone should have healthcare paid for, thus, this does not explain why the difference in average age at death. If the option had said the quality of healthcare in wistar is better than anywhere else then that would explain and weaken the conclusion.
B) The diets for people are similar for those in wistar v/s other regions so that cannot be a differentiator causing the average age to go up, thus strengthening that it was the mountainous aspect.
