Even if you are not sure on discloses/disclosed, check other parts of the given choices. Either parallelism is missing on  the number of  or correctverbs.

C is correct because releasing of report every first friday of the month is a continuous process.

Also in D

(D) disclosed the number of new jobs that  are  created during the reporting period and the number of hours being worked - 'are' is incorrect. 'the number of' is always singular.

The bold part states that an action happens every month so we need present simple tense. Hence D and E are our. Among A,B,C C wins since new jobs are countable nouns and hence "the amount of " is incorrect!

D using "number of hours being worked" being is used for on-going action so its use is wrong here..D out moreover, D isnt precise. one can say number of new jobs created rather number of jobs that are created

Split 1 - The sentence states a general fact- a report is released every friday; thus, we need "discloses" Eliminate (D) and (E)

Split 2 - SV Agreement - Hours and jobs are countable nouns. 1 hour, 2 hours 3 hours etc... therefore we need "the number" of hours and 
(A) discloses the  amount  of new jobs to be created during the reporting period and the amount of hours to be worked

(B) discloses the  amount  of new jobs being created during the reporting period and the amount of hours worked

Though I did this problem correctly, I want to stretch the discussion a little bit.

What if problem had started like this...Released on first friday of every month till 1990, the report disclosed/disclose....
I think in this case "disclosed" is correct, considering time frame mentioned in statement.

Am I correct?

I think "to be created" is a future action . this future action is not fit inhere. we need a simple action " jobs which are created during the reporting period". we need "are created" because "reporting period" require simple present.
