Two issues that can get you to correct answer...
1) BOTH..... correct usage is both X and Y..
2) THAT... use of that is important after 'revealed'..

Parallelism error. spritualist is not parallel to affected. also is also unnecessary.

B is incorrect because of and also affected. And & also placed next to each other causes redundancy.

C, D & E have more than one error.

Let's validate C - this creature....was a spiritualist and that he was..... In this sentence we have a parallelism marker "and" . X and Y. Both X & Y should be parallel.

D - has parallelism error as in C. It also has idiom error " both X and Y". But in D it uses "both X as well as Y" This is incorrect.

E - has parallelism error of C and Idiom error of D.

A is the best of All.

Can anyone share the role of "affected" here, if it is a Verb-ed modifier then what is it modifying

straight- no doubts about that. 'affected' modifies the noun just in front namely spiritualist. The spiritualist didn't affect anything. He was affected(was influenced) by his belief
