The correct answer is (A): believed to be.

The sentence is in the present tense, so the verb believed should also be in the present tense. The other options are either in the past tense or in the subjunctive mood, which is not correct in this context.

Here is the sentence with the correct verb tense:

The quartzite found in the Santa Ana Mountains is considered one of the most pristine Paleozoic mineral deposits, believed to be metamorphosed since the deposition of the Triassic era.

The other options are incorrect because:

(B) and it is believed to have been uses the past perfect tense, which is not correct in this context. The sentence is in the present tense, so the verb believed should also be in the present tense.
(C) and that is believed to be is wordy and awkward.

The underlying part should modify the quartzite hence D and E are out.
Looking at A it eliminates which --> also out.
between b and c, b seems to be correct because of the tense

B - believed to have been - it is believed to have been - correct
C - antecdent of that is the quartzite or deposits? ambiguous
D - structure issue + it antecedent ambiguous
E - subject verb agreement - deposits is not the subject therefore were is incorrect.

Therefore, Option B is the correct answer.

My answer: B

Option A: believed to be incorrectly modifies paleozic mineral deposits instead of the quartzite.
Option B: The use of the word and correctly brings back the focus on quartzite. Hence, correct.
Option C: and that is the incorrect usage to refer to quartzite.
Option D: which was is the wrong tense. The use of the word found indicates tense should be present.
Option E: are is not correct since quartzite is singular, not plural.

The quartzite found in the Santa Ana Mountains is considered one of the most pristine Paleozoic mineral deposits, believed to be metamorphosed since the deposition of the Triassic era.

(A) believed to be - Ed modifier modifies noun and it should modify "quartzite" but since it is placed too far away from the noun, meaning is lost.
(B) and it is believed to have been - correct, the word "since" in the sentence signifies that something started in the past and is still continuing, thus present participle is required.
(C) and that is believed to be - "That" can't refer to quartzite if there's a verb in between the two.
(D) which was, it is believed, - "which" can't refer to quartzite if there's a verb in between the two.
(E) which are believed to be - this seems to imply that the Paleozoic mineral depositis are believed to be metamorphosed...

the use of "and it is believed to have been" clarifies the subject and verb relationship. The sentence now correctly conveys that the quartzite is considered one of the most pristine Paleozoic mineral deposits, and it is believed to have been metamorphosed since the Triassic era.

IMO the correct answer is B.

(A) believed to be
This option is incorrect since 'believed to be' seems to modify Paleozoic mineral deposits.

(B) and it is believed to have been
This option is correct as it perfectly conveys the intended meaning and is free of grammar mistakes. 'it' has a clear antecedent, which is the subject of the independent clause (The quartzite).

(C) and that is believed to be
what is 'that'? Not clear. Eliminate.

(D) which was, it is believed
This option tries to incorrectly modify Paleozoic mineral deposits, using, in addition, incorrect form of verb

(E) which are believed to be
This option tries to incorrectly modify Paleozoic mineral deposits. It is The quartzite that sould be modified

'Believed to be' is talking about the subject of the sentence, which meant to be 'The quartzite'. B & C are too cumbersome. Between D & E, the subject of this clause is plural. E it is.
