My answer is (D). It took me 01:06.
(A) No justification for the use of continuous tense ("was increasing") to emphasize the ongoing nature of the action.
(B) Not a sentence. "IC + Comma + And + IC" is introduced by conjunction "since" as a subordinate clause, but there is no main clause.
(C) Not s sentence, "with A, and with B" is used as modifier, but there is nothing for it to modify, and there is no main clause whatsoever.
(E) Conjunction "because" should be followed by a clause, but " the percentage of 12- to 17-year-olds who reported marijuana within the last month increasing some 4 percent from 1990 to 1995" is a phrase.
