coming back to question
the sentence here is addressed in past tense "Dressed as a man and using the name Harvey Spector" so the complete sentence has to be similar tense.
A. 21, was injured four times, and was discharged in 1753, because she had become : what we have here is parallelism : was ...and,was along with this here tense tone of the sentence is past tense : correct choice
B. 21, was injured four times, while being discharged in 1753, because she had become. : we have a meaning issue here.. how can Donna be injured while being discharged.. does not make sense wrong choice
C. 21 and was injured four times, and discharged in 1753 because she was : sentence here lacks parallelism around "and" also use of she was is incorrect
D. 21, injured four times, and was discharged in 1753 because she w same issue as in option C
E. 21, having been injured four times and discharged in 1753, being : meaning issue ; having been injured.... being is wrong here
