GMAC has publicly stated that the Official Score that you earn on Test Day is within +/- 30 points of actual ability. Assuming a similar 'swing' in how your CATs function, your 4 CAT score results show that you essentially performed the same each time (+/- a few points) - and you're right at the proper ability level to hit your score goal. Unfortunately, at higher and higher score levels, the GMAT becomes really 'sensitive' to little mistakes. If you make to many of those mistakes, then a 720+ score simply becomes impossible to achieve. This is meant to say that you could probably find missing points in BOTH the Quant and Verbal sections, so limiting yourself to just the Verbal section might not be the best move. In addition, you have to make sure that you're taking your CATs in a realistic fashion.
When you took your CATs:
1) Did you take the ENTIRE CAT each time (including the Essay and IR sections)?
2) Did you take them at home?
3) Did you take them at the same time of day as your Official GMAT?
4) Did you ever do ANYTHING during your CATs that you couldn't do on Test Day (pause the CAT, skip sections, take longer breaks, etc.)?
5) Had you seen any of the questions BEFORE?

You are right about being able to find missed points in Quant. I've already gone through all my test mistakes and I've noticed there are anywhere from 4-6 careless mistakes I could have avoided on each Quant section. At a second glance, there were also about 2-3 questions on each test I could have solved instead of guess on - the answers to these were fairly obvious which is the worst part.
Of course I can't really say that my score could be X points higher since it's not the same conditions as the test I'm reviewing these questions in.
In this case I'm not really sure what else I can do. I noted these mistakes after each test, but it seems like just the time pressure is enough to make this happen. However I also sort of gambled on some DS questions I got wrong, going against my gut - these were mainly the "C" trap questions.
As for your question about CATS.
The first 2 were in different conditions than the GMAT. I took them at home and had to pause in the middle of the verbal section for the 2nd one since I had a really bad headache that day, so I came back to it after a short break. I think I may have seen 2 or 3 verbal questions before, but I didn't remember the answers to them anyway so I solved them as I would any other question.
The last 2 were also a bit different than the real deal, but I did them in the library with the same 8 minute break in between sections. They were just not at the same time as my real test. Also I did the entirety of all the exams (AWA to Verbal for all 4). In exam 6, 2 or 3 of the quant questions were very similar to ones on exam 5, just with different numbers/variables. There was also one repeat verbal question I noticed.
