Your profile is very promising, with your experience in Supply Chain planning and automation at the feeder brand Amazon capturing my attention the most. That, coupled with the problems you've solved in key projects, should be the center of your application.

You have not stated your goals. Your application should describe your long-term motivation and the problems you've recognized through your experience that you aim to solve with the expertise you'll gain over time. Consider the big picture. Allow your goals to be both aspirational and motivating. This could be a fun exercise! A well-thought-out approach would be to consider what you want to do in the long run, say, 10-15 years from now, and why. Then, divide your journey to your long-term goals into various alternative professional pathways (a Plan A and a Plan B) to assist you in selecting the relevant skills.

Since you have experience in planning and operations, I assume you could be interested in a strategy role/consulting or a leadership position with a global or high-growth startup. If planning and process design is your thing, you can also pitch a Head position in supply chain network design or in-house product management if you want to expand on your experience. Make a logical connection between your significant technical and supply chain knowledge and the types of projects you would work on in the short to medium term at the companies you want to work for. This is critical for a good ISB and IIM A interview.

General feedback: you would be a relatively senior applicant at ISB. Your career progression is the most important aspect on which B-schools will judge you. So, make sure you showcase this excellently in your essay and resume (work experience section of the ISB application). How have you shown an upward trend in the last ~10 years? A doctor wanting to do an MBA, Business schools will love to have you in the program. The best target score for any school is the average GMAT score of the current class, though you can get through with lesser than an average score of the class. However, average GMAT should be your target considering that you have cleared one of the most difficult admissions tests for your MBBS seat.

ISB is going to evaluate your profile on primarily 4 factors- 1) Academic Potential 2) Work Ex 3) Diversity 4) Career goals

Past scores show good academic potential and try to build on it by getting an equally good score. While in your case there are other strengths that can help you get in with a lower score, a 700+ will put you in a strong position. (for EEO a high score is anyway recommended as you will have less to talk about around work ex)
Work ex is missing and one will have to rely upon the internship and whatever you do in the remaining months. Just keep in mind that unlike academics, work ex is tougher to measure and what matters here is a) career progression b) achievements c) how you choose to present these in the application (essays, interviews and LORs)
Diversity (an ISB favorite) is what value will you add to the experience of your batchmates inside and outside the classroom. While the medical background adds diversity for sure, extracurricular also seem to show good potential. Just like work ex, presentation will matter here too.
Career goals, lastly- ISB wants to see leaders with a clear vision of their career and PGP as a stepping stone to fulfilling that vision. While your goals seem clear and organic, ISB will be curious to know why now and fit with the school (AKA your research and personalized reasons to attending ISB, and why EEO, why not later).

Focus on tying the above well in your application (apart from a strong GMAT score) and you have a decent shot.

Post that I went on to pursue Civil Services Examination but was unable to clear it.

You can also showcase experience and leadership skills to demonstrate that you are a valuable candidate, even if you come from an unfamiliar school. Including relevant certifications, highlighting extracurriculars and quantifying work experience, and achievements will add more value.

You must aim for a GMAT score of 700 or higher, if you are targeting schools such as ISB and IIM Ahmedabad , Bangalore, and Chennai for their executive programs. If you have a passport, you can also include some international schools in your list of targets. Just remember to quantify your achievements, experiences, and skills throughout your application to make it more professional and impactful.

Use your internships and academic projects to figure out precisely what areas interest you the most. For young applicants applying to YLP, EEO, or deferred MBA programs, it's critical to be thoughtful and specific while demonstrating your understanding of the industry you want to be a part of. You must present your plan so that the admissions committees, especially at the top US deferred MBA programs, will be rooting for you to succeed. Otherwise, your application will fall flat.
