Nationality/Age/Sex: Indian/31/Female
Degree: MBBS graduated in 2011. (Distinction in Anatomy, Forensic Medicine)
Worked as Casualty Medical Officer for an year
-Joined Post graduation: Joined DNB in Cardiothoracic & Vascular Surgery -- discontinued after an year
-Joined a Virtual medicine based healthcare company(pvt ltd) as a Consultant 3 years
-Got selected in Women Startup Programme at IIM Bengaluru(NSRCEL) for a 3 week Bootcamp Session
-Joined a US Healthcare company as Team Lead in Operations (9months)
-Joined a Hospital Group as Senior Manager in Business Development (2 months)
-Joined back my previous US Healthcare Company as Assistant Manager in Operations (present)
-GMAT: 650(Q: 46 / V: 34 / IR: 6)
-Work Experience: as above
-Post-MBA goal: Consulting

Your profile is strong and diverse in terms of your prior experience.
GMAT is certainly on the lower side. It may not exactly be a deal breaker in your specific case, but doesn't help a lot.
Not sure how your career goals tie in with your past skills and how are you planning to make this transition- I see this narrative as the biggest question mark based on the info present.
If you can tie in the admission factors mentioned here with career goals mentioned here in your written application, you may have a decent chance.
The timeline also looks a bit challenging unless you have not started work on the essays already.
