Hi there, Thanks for reaching out! Here are some thoughts on your questions:
1) Your GPA is on the low end but your GMAT is very strong, even among your demographic (which tends to have high GMAT scores). It also sounds like you've had strong accomplishments at work & that matters more than undergrad GPA. And you were facing medical issues during college so I'm guessing that's a big factor in the GPA? Assuming it was, you can use the optional essay to explain & the adcom will understand.
2) It's hard to say here since we need to know more about you but rest assured that there are plenty of applicants who get into these schools who have not had direct reports. It's all in how you talk about the leadership experience you HAVE had, which is certainly not limited to having people report to you. This is something we work through with our clients in detail so although I can't really give you specifics here on the approach we'd recommend for you (just because I'd need more info) rest assured that it is possible!
3) The main thing is that you need to show what's changed since you applied last time. That might include new accomplishments at work & can also include any refinements to your goals. When we work with clients we take a look at past applications so we can ID what went wrong the first time & figure out a plan for how to proceed with the new app. (Speaking of goals, we would need to narrow them down to ONE path. You want to show a targeted focus since your argument for why you need the MBA rests on your goals. You will have slightly different skill gaps to fill depending on which path you take).
4) Yes, it might be worth broadening your reach to top 10 just because you are coming from such a competitive demographic (the toughest one when it comes to b-school applications) but you definitely have a shot at these schools assuming strong essays & an overall strong story that illustrates YOUR unique value prop (i.e. why they should admit you vs. someone else from your demographic with equally impressive stats)
5) At this point it's too late honestly to improve your profile. As an international applicant you want to apply R1 to as many schools as possible (even more important when you are coming from a competitive demographic) so the best thing you can do right now is a) refine & finalize your post-MBA goals -- not just what you want to do but WHY and 2) dive into those applications so you can do your very best work on the essays & get as many apps in R1 as possible!
