Hi - I just took the Official Practice Test (just the quant) and scored a 41, which is obviously terrible because it's in the 39%ile.
I didn't study at all (just took it cold) and haven't done this kind of math in ages. I don't have a job that uses numbers at the moment.
What sort of study plan should I pursue? Any advice? What is a realistic score that I can achieve from this base?

Hey kr22, Welcome to GMATClub.
It is a good thing that you took a cold official mock, but no expert will be able to completely help you until you tell your complete score i.e. Quant score, verbal score, IR score.

Magoosh also has a couple of study plans that are really popular, maybe you can check those out on Magoosh's website.

Hi kr22, Q-41, without studying, cannot be terrible, and it is indeed a great start. Working with concepts from Manhattan prep all the quant book can help you to familiarize problems and score better.
Search 'The Definitive GMAT Study Plan - 2022 Edition', you can have some good stuff.
