I took my Gmat exam on 29 September 2016 and scored a 570 (Q36, V33). I would like to improve and hope to increase score upto ~670 in order to meet the median scores of my target programs.

Quantitative Preparation: I have finished the
MGMAT Quants guides thrice (learning and revising) now and feel that the know that material well. Moreover, I've finished solving about 90% of the OGmat guide, noting problems which I got wrong.
In the quantitative section I feel that
Data sufficiency pose's a serious challenge for me (specially Inequalities DS type problems). I requested a Enhanced Score report, but wasn't able to gleam a lot of actionable information from it ( I was sincerely hoping that experts here could please take a look at it and instruct me further).
Verbal Preparation: The biggest challenge is the Sentence Correction module of the exam.
Currently, I've been using my 'ear' to find answers to SC problems. But I really want to improve SC by learning the rulesets, identifying various components of the sentence to find the best answer. Please help me formulate a methodology to do so. I would not be able to apply for Round I of admissions of my target programs because of my dismal performance. The deadline for R II is around 21 November. That give's me about ~40 days to prepare for my second gmat test (please note that I largely have my applications ready and just need to polish them before sending. Gmat is the biggest challenge right now).
Some background: I recently had some surgery and my lower back/posterior has a slight twinge, if i sit for an extended period of time. This used to hamper my level of concentration, but I have been able to soldier through the pain. I am between a rock and a hard place right now because I've chosen a challenge such as the Gmat (and not performed well) during my period of recuperation. I would really appreciate if someone here could guide me on aspects of improving my 'test-taking skills/abilities' (during my exam I noticed that I began to fidget a little during the last portion of my test, Enhanced score report confirms that there was a drop in the the number of incorrect answers at that point in time).
Moreover, are there
'good-habits' with regard to test-prep, which may directly impact my study and help improve score ? I am quite flexible with regard to the number of hours which I can devote towards gmat preparation, but the number of days is limited to 40 for me to skill-up.
